5 Why Analysis: Simple Truth

When a defect happens, we often stop here:
... Worker was careless
... Human error

Case closed. Problem repeats....But real 5 Why goes deeper:

Why?  Rushed work
Why?  Unrealistic targets
Why?  Pressure from management
Why?  Fear of speaking up

So root cause: the system, not the worker!
